Event Description
It was reported that a patient underwent an atrial fibrillation - persistent ablation procedure with a thermocool® smart touch® sf bi-directional navigation catheter and the following errors were noted: error (106) force catheter sensor error and error (2604) force value missing the catheter could not be zeroed.A new cable did not solve the problem.The catheter was pulled out of the heart and it was seen that there was blood in the tip of the catheter.The blood ran under the transparent cover.The tip could also be bent and you could see the tip breaking through.The "force spiral" was broken.There was no difficulty in removing/maneuvering the catheter from the patient's body.A new catheter was used for the procedure.There was a 10 minute delay to resolve the issue.The procedure continued with the new catheter and was successfully completed.No patient consequences were reported.
