It was indicated that the device will not be returned for evaluation.If there is any further relevant information obtained, a supplemental medwatch will be filed.
 
Event Description
It was reported that during procedure a faradrive steerable sheath was selected for use, however after the faradrive sheath was introduced to the patient after transseptal puncture and the dilator was removed along while wire was kept inside, the physician started aspirate through the faradrive sheath but air bubbles kept getting out of the device handle and into the three-way stopcock/syringe, therefore it was decided to replace the device and the issue was resolved.The procedure was completed.No patient complications were reported.The device is not expected to be returned; it was disposed at facility.
