Three attempts were performed to obtain additional information, but no response was received from the customer.The device was returned to olympus for evaluation and the customer¿s allegation was confirmed.The device evaluation found the connector pins were broken and the transducer cannot be plugged in.Based on the results of the investigation and information obtained from this complaint, the cause was traced to a component failure.A device history review revealed findings/no issues that could have caused or contributed to the reported issue.Olympus will continue to monitor the field performance of this device.Should additional relevant information become available, a supplemental report will be submitted.
