Event Description
|
It was reported that during the implant attempt of the cardiac resynchronization therapy defibrillator (crt-d), the right ventricular (rv) lead pin could not be inserted into the connector port.It was felt that the lead tip was bending upward and not going in straight; it may have been in the left ventricular (lv) port.The clinician also noted noise and high impedance issues.The device was not used and a new device was implanted which resolved the issues.The lead remains in use.No patient complications have been reported as a result of this event.
