This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.The device was not returned to olympus for inspection.In speaking to olympus technical assistance center (tac), the customer was provided with troubleshooting methods.Upon continuation of the reported event, the customer was advised to send in the device for evaluation.However, they sent the scope to a third party for repair.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 11 years since the subject device was manufactured.Based on the results of the investigation, the cause of the reported issue is most likely due to water invasion.Additionally, a b30 scope error occurs when there's an abnormality in the communication between the endoscope and the processor.Olympus will continue to monitor field performance for this device.
