It was reported that "a ligated clip reopened when the user treated another blood vessel.Therefore, the user used another cartridge to re-ligate and completed the surgery.No clip fell/remained in the patient, and no injury to the patient occurred".Additional information received states that "[the user] was able to observe the moment it came off with an endoscope, so he noticed it right away.The blood vessel was immediately relegated with another clip to stop the bleeding.The incident was a case of gastric surgery, and the clip that was attached to the right gastroepiploic artery came off".
