Event Description
It was reported, the light source device that arrived back to the customer site from repair was still exhibiting error code b30.The issue occurred during receipt inspection.Initial troubleshooting was performed via phone call with the customer to try and isolate the reported issue.The customer was informed that the device would need to be sent in for evaluation and repair.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.Additional information added to fields: d8, h3 and h6.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over one (1) year since the subject device was manufactured.Based on the results of the investigation, it is likely the following led to the malfunction: we surmised that b30 error occurred due to faulty scope socket.B30 error occurs when an abnormality occurs in the communication between the endoscope and the processor.There was no fact indicating that the cause was a misuse.Olympus will continue to monitor field performance for this device.
