Event Description
It was reported the tip of the rigid endoscope sheath broke during the cystoscopy with transurethral resection of bladder tumor procedure.The tip was retrieved from the patient.The procedure was prolonged approximately 30 minutes and then completed with a similar device.There was no reported patient harm.
