The device was not returned for analysis.Lot history record (lhr) and corrective and preventive actions (capa) reviews were performed and revealed no indication of a product quality issue.Additionally, a review of the complaint history identified no similar incidents from this lot.The reported patient effect of dissection is listed in the xience alpine everolimus eluting coronary stent system (eifu), electronic instructions for use (eifu), as a known patient effect of coronary stenting procedures.A conclusive cause for the reported patient effect, and the relationship to the product, if any, cannot be determined; however, the subsequent treatment appears to be related to the operational context of the procedure.There is no indication of a product quality issue with respect to manufacture, design or labeling.
 
Event Description
It was reported the procedure was to treat a mildly calcified lesion in the right coronary artery (rca).A 2.75x28mm xience alpine stent was implanted in the proximal rca and a 2.75x38mm xience alpine was implanted in the mid to distal rca.During post dilatation with a nc balloon, a perforation occurred in the distal rca.A 2.80x19mm graftmaster was implanted to seal the perforation.There was no clinically significant delay in the procedure and no adverse patient sequela.No additional information was provided.
