Event Description

A healthcare professional reported that during cataract surgery cassettes were not being accepted by an ophthalmic system, and an ophthalmic handpiece and tip were blocked in phacoemulsification mode.Surgery was completed by changing the cassette and by restarting the system.Patient experienced corneal burn and whitening of the cornea.
