Event Description
It was reported, that the high flow insufflation unit turned on without making any sound when started up, but the entire panel went dark and the alarm continued to ring.It was not immediately reproducible, but it was reproduced after restarting it about 20 times.The issue was found during preparation for use for a therapeutic laparoscopic nephrectomy that was completed with a similar device.There were no reports of patient harm.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the device evaluation and the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over eight (8) years since the subject device was manufactured.The device was returned to olympus for inspection, and the customer's reportable malfunction was confirmed.Based on the results of the investigation, a definitive root cause could not be determined.However, it is likely that all panels are off and alarm sounds continuously occurred due to the light emitting diodes other than the power supply was turned off and the insufflation operation stopped while the alarm sounded, when an error was detected in the pressure sensor on the main printed circuit board.Olympus will continue to monitor field performance for this device.
