Event Description
|
It was reported from mexico that during an unspecified surgical procedure, it was discovered that the truespan 0 degree peek device was passed to the infirmary and graduated to 20mm depth.The doctor took it and introduced the system to place it in the meniscus, then introduced the needle and proceeded to shoot it with the red trigger.However, the system encountered resistance, and when some force was applied, the mechanism broke and became unusable.It was reported that there were no delays to the surgical procedure as a spare device was available to complete the surgery.There was patient involvement reported.There were no reports of injuries, medical intervention or prolonged hospitalization.All available information has been disclosed.If additional information should become available, a supplemental medwatch report will be submitted accordingly.

This report is being submitted in pursuant to the provisions of 21 cfr, part 803.This report may be based on information which has not been able to investigate or verify prior to the required reporting date.This report does not reflect a conclusion by depuy mitek or its employees that the report constitutes an admission that the device, depuy mitek, or its employees caused or contributed to the potential event described in this report.If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.Device was used for treatment, not diagnosis.Investigation summary: the product was returned to depuy synthes mitek for evaluation.Depuy synthes mitek then conducted visual inspection of the device received.The complaint device was received and evaluated; visual inspection reveals that the red trigger is broken.The plates and the suture were found still attached to the needle.The device was opened to verify the broken part, it was found that the upper part of the red trigger was broken.The applier needle was found in a normal condition functional test was unable to be performed due to the condition of the device.The overall complaint was confirmed as the observed condition of the truespan 0 degree peek would contribute to the complained device issue.Based on the investigation findings, the potential cause can be traced to a portion of tissue that was entered inside the applier needle causing a mechanical obstruction of the first plate when it was going to be deployed; this obstruction and the force applied to the trigger are contributive factors of the broken trigger.Therefore, it has been determined that no corrective and/or preventative action is required.As part of depuy synthes mitek quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history review : a review of the batch manufacturing records was conducted on finished lot number 133l381: and no related non-conformances were identified.
