The hospital reported that during an endoscopic vein harvesting procedure, vasoview hemopro vh-3500 during branch ligation, it was noticed that the c ring was broken.One of the plastic "arms" that extends out of the harvesting cannula to hold the c ring was broke.When the harvester noticed this, the device was immediately removed from the patient.At this time, it was also noticed that the end of one of the jaws of the harvesting tool was also broken.It appears that the c ring and the harvesting tool may have become stuck together at some point and this may have caused the defects.Per photos provided by the complainant, the c-ring has broken off at the end of the device, and there is evidence of blood.No injuries occurred due to the defects and no parts of the device were lost during the case.The defective kit was removed from the surgical field and a new device was opened up.The only surgical delay was the time needed to open a new kit, which was about 2 to 3 minutes.The remainder of the case was finished with no further issues.
