Event Description
It was reported that a patient underwent a ventricular tachycardia ablation procedure with a thermocool® smart touch® sf bi-directional navigation catheter and the patient experienced cardiac perforation that required pericardiocentesis and surgical intervention.During the procedure, the patient's blood pressure started to drop.They checked for effusion and baseline effusion was growing.Pericardiocentesis was performed to drain blood from the pericardial space.Patient wasn't able to be stablized and continued filling.The patient was sent for open heart surgery.Suspected perforation with ablation catheter in the right ventricle (rv) from force (ablation only performed in the left ventricle lv not the right ventricle).High force occurred for breath on anterior free wall of right ventricular outflow tract (rvot) when the ablation catheter entered the rv.Hole.5 x.5 in anterior free wall rvot was found during surgery.Catheters did not seem to fail.
