It was reported that the patient with a cardiac resynchronization therapy defibrillator (crt-d) and a right ventricular (rv) lead showed a low heart rhythm in the thirties and a syncopal episode.When the rv automatic threshold test was analyzed, two different morphologies were present with what appears as loss of capture.It was recommended to turn off trend, increase the pacing threshold and monitor the patient.The crt-d and the rv lead remain in service.No additional adverse patient effects were reported.
