The health care professional (hcp) requested technical services (ts) review of device data to confirm device operation.It was noted that the patient with this cardiac resynchronization therapy defibrillator (crt-d) received inappropriate anti-tachycardia pacing (atp) and inappropriate shocks.It was noted pacemaker mediated tachycardia (pmt) episodes were also recorded.Upon review of the presenting electrogram (egm) showed that the inappropriate therapy and pmt episodes appeared to be due to atrial fibrillation (af) with rapid ventricular response (rvr).It was noted that the shock therapy had difficulties converting the rhythm.At this time, the crt-d remains in service.No additional adverse patient effects were reported.
