Event Description
It was reported that the cyto-nephro videoscope was not correctly reprocessed, the water supply pipeline of the reprocessor had never been disinfected, and the water filter had never been changed after delivery to the user facility.The issue was found during reprocessing.There were no reports of patient involvement.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over seven (7) years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause could not be determined as the device was not returned for evaluation.Although, the customer confirmed that the device was incorrectly reprocessed it cannot be specified as to why.The event can be detected/prevented by handling the device in accordance with the following instructions for use (ifu): instruction manual endoscope cleaning and disinfection equipment oer-s(acecide 6% for disinfectant)operation edition; chapter 3 inspection before use; chapter 4 basic procedure of cleaning and disinfecting endoscopes; chapter 6 other functions.Olympus will continue to monitor field performance for this device.
