Event Description
|
As reported, during an unknown procedure, the coating of a roadrunner uniglide hydrophilic wire guide ¿curled¿.Latex free gloves were worn, and the coating was activated with normal saline.The wire was used four-to-five times.The wire was not kept hydrated when not in use but was reactivated before re-use.The anatomy was not tortuous, scarred, or calcified.Resistance was not encountered upon insertion of the wire guide.Then wire was not removed through a needle or other metal instrument.Reportedly, the coating was uneven and not smooth and curled in the middle of the wire ¿like waves¿.No metal was exposed.A section of the device did not remain inside the patient¿s body.The patient did not require any additional procedures due to this occurrence.According to the initial reporter, the patient did not experience any adverse effects due to this occurrence.
