Event Description
It was reported that tubing of a clearlink secondary medication set was falling out of a non-baxter solution bags.As a result, an unknown medication spilled out of the bag.The customer also stated that she believes the issue is with the non-baxter bag and not with the tubing as the facility has not experienced the issue, since they have stopped using the non-baxter bag.There is no report of patient/user injury or medical intervention was needed in association with this event.No additional information is available.
