The autopulse platform in complaint was returned to zoll on (b)(4) 2014 for investigation.Investigation results as follows: visual inspection of the returned platform shows that the display screen was not working.It was found that the lcd was defective.Additionally, the head restraint wire was cut/broken, the front enclosure was cracked, and the battery lock was bent.These physical damages are not related to the reported issue of the display screen not working.The damages appear to have been caused by normal wear and tear (autopulse manufactured in march of 2008) or mishandling of the platform.Review of the archive was performed and no problems were found.Functional testing was performed and the platform passed testing.The platform ran for 7 minutes with the test mannequin and no problems were observed.Based on the investigation, the parts identified for replacement were the lcd display, top cover, battery lock and the front enclosure.In summary, the reported complaint of the display screen not working was confirmed during visual inspection.The fault was found to be due to the defective lcd display.Upon replacement of the lcd display and the damaged parts, the platform passed all testing criteria.
