Event Description
It was reported by a physio-control service representative that a loaner device would continually reset during the boot-up sequence.This was observed during testing and there was no patient use associated.
 
Manufacturer Narrative
Physio-control evaluated the device and verified the reported failure.Physio replaced the flex cable assembly which connects the user interface pcb to the pcb stack and observed proper device operation through functional and performance testing.The device was then returned to the customer for use.The removed flex cable assembly was further evaluated in the failure analysis center.It was observed that the flex pads from flex cable connector, designator p21 were broken away from the solder at the pins.
