Event Description
It was reported that the procedure was to treat a 99% stenosed lesion in the mid to distal right coronary artery with mild calcification.Pre-dilatation was performed.A 3.0 x 15 mm xience xpedition stent was implanted distally; however, a dissection occurred.The 3.0 x 23 mm xience xpedition stent delivery system (sds) was advanced to treat the dissection; however, the sds failed to cross and the hypotube separated outside the anatomy.A new xience xpedition sds was used to treat the dissection successfully.There were no adverse patient effects and no clinically significant delay in the procedure.No additional information was provided.

(b)(4).Concomitant products: stent: 3.0 x 15 mm xience xpedition; guide wire: route; guide cath: brite tip 6f jr4.0.The 3.0 x 15 mm xience xpedition referenced is being filed under a separate medwatch report.Evaluation summary: the device was returned for analysis.The separation was able to be confirmed.The failure to advance could not be replicated in a testing environment as it was based on operational circumstances.Based on a visual and dimensional inspection of the returned device, there is no indication of a product deficiency.A review of the lot history record revealed no non-conformances that would have contributed to the reported event.A query/review of the complaint history of the reported lot did not indicate a manufacturing issue.Based on the reviewed information, no product deficiency was identified.
