It was reported patient underwent a comprehensive reverse total shoulder arthroplasty on (b)(6) 2012.Subsequently, patient was revised on (b)(6) 2015 due to a fractured screw caused by a malpositioned baseplate and stem.During the procedure, infection was noted.All components were removed and cement spacer molds were implanted.The fractured screw remains in the patient.There was a reported delay of 40-45 minutes due to the stem being well fixed and difficult to explant.

Current information is insufficient to permit a conclusion as to the cause of the event.Review of device history records show that lot released with no recorded anomaly related to the event.There are warnings in the package insert that state that this type of event can occur: under warnings it states, "improper selection, placement, positioning, alignment and fixation of the implant components may result in unusual stress conditions which may lead to subsequent reduction in the service life of the prosthetic components." and " fatigue fracture of component may occur as a result of loss of fixation, strenuous activity, malalignment, trauma, non-union, or excessive weight." and "early or late postoperative infection and allergic reaction.".
