Event Description
|
It was reported to boston scientific corporation that an rx cytology brush was used in the duodenum during an endoscopic retrograde cholangiopancreatography (ercp) procedure performed on (b)(6) 2015.According to the complainant, during the procedure, the brush was stuck in the ampulla and it would not advance into the duct over the wire due to blunt tip.The procedure was completed with another rx cytology brush.There were no patient complications reported as a result of this event.The patient's condition at the conclusion of the procedure was reported to be okay.

(b)(4) difficulty advancing into the duct.Although the suspect device has been received, the evaluation has not been completed.Therefore, the cause of the reported malfunction has not been determined.Upon completion of the failure analysis of the complaint device, if there is any further relevant information from that review, a supplemental mdr will be filed.

Visual evaluation of the returned device revealed that the catheter working length was bent and twisted in multiple places throughout.The catheter tip was undamaged, its lumen openings and distal edge were in good condition.The brush was undamaged, properly formed and was fully extended and retracted without issue during functional evaluation.The bends and twists in the device and the difficulty in engaging the target vessel were most likely due to some aspects of tortuous anatomy or other operational factors of the procedure.Therefore, the most probable root cause classification is "operational context." a review of the device history record (dhr) confirmed that the device met all material, assembly, and product specifications at the time of release to distribution.A search of the complaint database confirmed that no other complaints exist for the specified lot.
