It was reported that during the cardiac resynchronization therapy - defibrillator (crt-d) device implant procedure, noise was detected and there was a suspected "defect in the screw of the connector port." it was noted when the torque wrench was attempted to be used during connection, it "could not be used at the right angle and spun around in aimless circles." the physician also noted a "suspected non-conformity of the device due to the subtle difference with other manufacturer's standards." the crt-d was explanted and replaced.No patient complications have been reported as a result of this event.
