Event Description
It was reported that there was a leak from the twist clamp of a uv flash transfer set during use.There was no patient injury or medical intervention associated with this event.No additional information is available.
 
Manufacturer Narrative
(b)(4).The device was received for evaluation.A visual inspection was performed and holes were observed a hole in the tubing.Clear passage testing, clamp function testing and device-device interaction testing were all performed with no issues noted.Leak testing was performed and confirmed the reported issue.The cause of the issue was determined to be hole in tubing.Should additional relevant information become available, a supplemental report will be submitted.
