(b)(4).The results of the investigation are inconclusive since the device was not returned for analysis.Our investigation was limited to the review of the device history record, which showed that each manufacturing and inspection operation was performed and indicated complete in accordance with sjm specifications and procedures.Based on the information received, the cause of the reported incident could not be conclusively determined.
 
Event Description
The atrial septal defect measured 22 mm in two views and 21 mm in one view on toe.The septum was stable and not floppy.A 24 mm amplatzer sizing balloon ii was inflated to assess sizing and the waist on the balloon measured 24 mm.A 26 mm amplatzer septal occluder (aso) was selected.The aso was deployed and assessed on toe, it was noted at the aortic rim the left atrial disc had prolapsed through to the right side.The aso was snared but did not retract easily inside the 12f amplatzer torqvue 45 delivery system (dtv45) due to the angle of the screw.The aso eventually embolized into the pulmonary artery where multiple attempts were made to snare the device without success.A vascular surgeon performed a cut down in the superficial femoral vein and successfully removed the aso.
