(b)(4).Evaluation is in progress, but not yet concluded.The field service representative (fsr) verified that the arterial monitor on start up would reboot several times and then the display would go blank.During troubleshooting he found that the arterial monitor computer board was the source of the issue.The fsr replaced the arterial monitor computer board and checked arterial monitor calibrations and operation.The unit operated to manufacturer specifications and was returned to clinical use.The suspect part was returned to the manufacturer for further evaluation.
 
Event Description
It was reported that during set-up of the device for a cardiopulmonary bypass procedure, when the arterial monitor powered up, it constantly rebooted then went blank.No audible alarms were heard.As a result, an alternate device was employed.The surgical procedure was completed successfully.There was no delay, no blood loss, nor adverse consequences to the patient.
 
Manufacturer Narrative
The reported complaint was confirmed.During laboratory analysis, the product surveillance technician (pst) installed the returned board into a lab use only cardioplegia monitor.When power was supplied, the monitor¿s display would illuminate for a second and then not illuminate.Occasionally, the monitor would reboot but then the display would not illuminate.If additional information becomes available on this complaint that would alter the facts and/or conclusion, a supplemental report will be filed accordingly.
