Event Description
The patient reported the desktop charger connector broke off.It was not an out of box failure.It was further reported the replacement device sent to the patient was incorrect and the patient was in pain.The patient saw the arrow piece and said to forget she called since she took care of it.The patient was going to charge as soon as she could.The indication for therapy was spinal pain.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.A good faith effort will be made to obtain the applicable information relevant to the report.If information is provided in the future, a supplemental report will be issued.
