Event Description
|
It was reported a hemostasis valve leak occurred and air appeared in the device.The patient was undergoing a left atrial appendage (laa) closure procedure.A watchman® access system was inserted into the patient; however, the hemostasis valve was not sealed which created a leak of blood.A watchman® laa closure device and delivery system was introduced in the access system, but because the valve was closed to stop the leak, the delivery system strongly rubbed and kinked the access system.Then air was noticed in the access system, so the physician aspirated the air and removed both the access system and delivery system from the patient.A new access system was inserted into the patient and the same delivery system was inserted into the access system.The closure device was successfully deployed and there were no patient complications.The patient's condition post procedure was stable.
