On (b)(6) 2016, a gore acuseal vascular graft was implanted in patient's right thigh in an arteriovenous access procedure.On (b)(6) 2016, the patient presented for a declot procedure.A trerotola thrombectomy device was used in an attempt to declot the graft.The device was placed beyond the clot, and as the thrombectomy device was moved back and forth, sections of the inner walls of the graft was torn off.The frayed graft wall was pushed back into place with an implant of a gore viabahn endoprosthesis.The patient did not experience any adverse consequences.
