(b)(4): during processing of this complaint, attempts were made to obtain complete event, patient and device information.(b)(4).Evaluation summary: the device was returned for analysis.Visual inspections were performed on the returned device.The labeling issue was confirmed.The investigation determined the reported difficulties appear to be related to circumstances of the procedure.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other similar incidents from this lot.The investigation determined the reported difficulties appear to be related to circumstances of the procedure.Based on the information reviewed, there is no indication of a product quality issue with respect to the design, manufacture or labeling of the device.
 
Event Description
It was reported that the box of the 2.5x15 xience alpine stent was opened, but the aluminum pouch contained a 2.75x12 xience alpine stent.The device identification stickers on the box have both the 2.5x15 and 2.75x12 information.There was no patient involvement and no clinically significant delay in the procedure.Another xience alpine was used to complete the procedure without further incident.No additional information was provided.
