Current information is insufficient to permit a conclusion as to the cause of the event.Review of device history records show the lot released with no recorded anomaly or deviation.There are warnings in the package insert that state that this type of event can occur: under possible adverse effects, number 10 states, "fatigue fracture of component can occur as a result of loss of fixation, strenuous activity, malalignment, trauma, non-union, or excessive weight." device remains in patient.

Patient underwent a shoulder revision due to a fractured humeral tray.

This follow-up report is being submitted to relay additional information.Reported event was unable to be confirmed due to limited information received from the customer.Products were determined to not be compatible as competitor¿s products were used with zimmer biomet components.Record was reviewed and no discrepancies were found.Root cause is attributed to the device design.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
