Event Description
It was reported that the 5.0 x 80 6f 120 cm supera stent delivery system was prepped and flushed without issue; however, the tip was observed to be kinked and when touched by hand the tip fell off the device.The device could not be used on the patient.A new supera was used to complete the procedure without further issue.There was no clinically significant delay in the procedure reported.No additional information was provided.

(b)(4).Evaluation summary: visual inspections were performed on the returned device.The reported separation was confirmed.The reported kinked tip was not confirmed; however there was a kink proximal to the tip.A review of the lot history record revealed no manufacturing nonconformities that would have contributed to this complaint.Additionally, a review of the complaint history identified no other incidents from this lot.Based on the information reviewed, there is no indication of a product quality issue with respect to the design, manufacture, or labeling of the device.The investigation was unable to determine a conclusive cause for the reported difficulties.
