It was reported that during a cryoablation procedure, as the sheath was advanced into the body, the patient started groaning in pain. The competitor guidewire was seen in the left superior pulmonary vein (lspv), however, the sheath was not seen. The sheath had "hooked downward" onto itself within the groin. The anticoagulation was reversed and an angiogram was performed through the sheath. It was observed that contrast had extravasated into the pelvis. The case was aborted while the patient was not under general anesthesia and the patient underwent vascular surgery to repair the femoral vein. The patient is now doing well and no further patient complications have been reported as a result of this event.
