Currently, it is unknown whether or not the device may have caused or contributed to the event as no product has been returned.The device will be returned for analysis and further information will follow once the analysis has been completed.No conclusion can be drawn at this time.(b)(4).
 
Event Description
The customer reported via phone call of failed battery test, motor position encoder error, motor error alarm and exposure to high magnetic field from the insulin pump.The customer's blood glucose reading was 310 mg/dl.The customer treated with insulin pen.The customer stated that she received motor error alarm and failed battery test prior to calling.The customer stated that there is a black dot on top of the pump.The customer stated that the pump was exposed to an mri.Customer was advised to discontinue use of the device and to revert to a backup plan per health care provider's instructions.The customer will be sent a replacement pump.
 
Manufacturer Narrative
All operating currents are within specification.No unexpected failed battery test alarms noted.The insulin pump functioned properly.The insulin pump was received with constant motor error alarm during rewind due to motor encoder out of phase.We were unable to confirm alarm, unable to perform displacement test, or complete functional test due to motor error alarm.The insulin pump was received with cracked reservoir tube lip and cracked battery tube threads.
