This alleged failure mode poses a low risk to a patient because although the companion 2 driver exhibited single compressor malfunction alarms, the companion 2 driver was used for training purposes only.The companion 2 driver has been returned to syncardia for evaluation.The results of the evaluation will be provided in a follow-up mdr.(b)(4).
 
Event Description
The companion 2 driver was not supporting a patient.The syncardia systems team member reported that during on-site training, the companion 2 driver exhibited single compressor malfunction alarms when the beat rate was set to zero.
 
Manufacturer Narrative
The companion 2 driver was returned to syncardia for evaluation.Review of the patient file confirmed the single compressor malfunction alarms as reported by the customer.The single compressor malfunction alarm was reproduced and found to be caused by the left motor controller printed circuit assembly (pca).This issue will continue to be monitored and trended as part of the customer experience process.Syncardia has completed its evaluation of this complaint and is closing this file.(b)(4).
