Event Description
Information was received from a manufacturing representative (rep).Telemetry problem with 8840 was reported; the rep was in a case and the pump was out of the box on the back table, they were initially able to interrogate the pump and at the time of the call, they could not establish telemetry.There were no symptoms reported.The rep was going to try turning off the lights to see if that would help resolve the issue and call back if they could not resolve the issue.It was discussed that they can use the backup pump if they cannot establish telemetry on this pump again (pump was already in sterile field on back table and could not be moved).Additional information received from the manufacturer representative.It was reported that the pump was never implanted.They did not test again to see if telemetry could be established.

The pump (b)(4) was returned for analysis.Upon interrogation the pump was used to deliver water (1000 mcg/ml at 6.4 mcg/day).Analysis of the pump found no anomalies.The conclusion code (b)(4) no longer applies.A good faith effort will be made to obtain the applicable information relevant to the report.If information is provided in the future, a supplemental report will be issued.
