Event Description
The health care provider (hcp) via a manufacturer representative reported that when the lead was passed to the surgeon it was ok on (b)(6) 2016.While scoping after lead insertion and before suturing, the surgeon noted the blue monofilament was broken.The propylene was broken.The lead was replaced and would be sent back for analysis.No symptoms were reported.
