The full lead was returned and analyzed and no anomalies were found.A good faith effort will be made to obtain the applicable information relevant to the report.If information is provided in the future, a supplemental report will be issued.
 
Event Description
It was reported that the left ventricular (lv) lead dislodged during catheter slitting.The catheter and lead were removed.A second attempt was made to cannulate the coronary sinus when contrast dye showed a perforation.The patient subsequently had multiple episodes of ventricular fibrillation (vf) which were successfully treated with therapy from their defibrillator.Cardiopulmonary resuscitation was also performed.The perforation was repaired with an open heart surgery.No further patient complications have been reported as a result of this event.
