It was reported the physician was implanting a gore® cardioform septal occluder in a defect that was balloon sized to 15-16mm.The device was locked and fully released.On echo the physician noted a residual leak and decided to remove the device.He advanced a 11f long arrow sheath and snare to remove the occluder.After the removal, echo demonstrated the atrial septum was modified due to the removal of the occluder.The septum was sized to 21mm in diameter with the balloon.The physician attempted to close the defect with a 24mm aso.The case was unsuccessful.The patient was taken to surgery for repair of the defect on (b)(6) 2016.The patient was doing well following surgery and was discharged from the hospital on (b)(6) 2016.
