This is a report of a user error where the unused supply lines were not clamped off prior to starting therapy.Use errors and proper user instructions are addressed in ¿the homechoice and homechoice pro systems patient at-home guide¿, which is shipped with every homechoice device.The guide warns the user to make sure all clamps on unused fluid lines are closed securely when performing peritoneal dialysis therapy.The guide instructs the user to close all clamps while preparing to load the disposable set.A review of the label for the product family will be conducted.Should additional relevant information become available, a supplemental report will be submitted.
 
Event Description
It was reported that a homechoice cassette was leaking from an unused supply line.This occurred during set-up of peritoneal dialysis therapy.During troubleshooting, it was discovered that the unused supply lines on the cassette were not clamped off prior to the start of set-up for therapy.The technical services representative advised the patient to start over therapy with all new supplies and assisted with removing the cassette.There was no patient injury or medical intervention associated with this event.No additional information is available.
