Event Description
It was reported that the lv lead was turned off due to phrenic nerve stimulation in every pacing configuration.During device change out, a possible perforation was seen under fluoroscopy.The lead was capped and replaced on (b)(6) 2016.The patient was in stable condition post procedure with no complications noted and was doing well.
