A patient experienced a separation issue while using a baxter interlink continu-flo set which resulted in blood loss.A separation at the luer lock connecting anesthesia port manifold occurred after the customer had finger tightened the device.The device came apart and the patient lost a significant amount of blood (amount not specified).There were no visible abnormalities to the product or its packaging.No additional information was available regarding patient outcome.No additional information is available.
