The product was returned with the membrane completely unfolded and blood on the interior and exterior of the catheter and between the catheter and the sheath.The extension tubing was also returned.An underwater leak test of the balloon, catheter, y-fitting, extracorporeal and extension tubing was performed and one leak was detected on the membrane approximately 1.3cm from the rear seal measuring 0.025m in length.The reported problem was most likely triggered by a leak which was found on the membrane.Under magnification, a whitish patch was observed around the leak.This whitish patch is the typical appearance of an abrasion mark which is caused by calcified plaque in the aorta.(b)(4).
 
Event Description
Icu rn called to report they had removed an iab for blood in the catheter and tubing all the way up to the pump.It was sent to biomed.The patient had come to them several days ago with this balloon in place from (b)(6).They were told that 2 previous balloons had leaked in this patient and she is known to have extensive aortic calcifications.Patient developed hematoma in l fem p pressure for hemostasis.Went to cath lab at 1300 for new iabp placement.
