Event Description
It was reported that during implanting the lens, the trailing haptic "curled up" and looked "wavy." the lens was cut out intra-operatively and the doctor enlarged the incision and added 4 sutures.The same model lens was implanted with no issues.Physician's opinion of the most likely cause of the lens damage was "injector was defective.".

The lens was not returned, therefore a product evaluation could not be performed.The device history record was reviewed and there were no discrepancies or unusual finding that relate to the reported issue.The exact cause of the reported event could not be conclusively determined.However, it is possible that user related factors (such as loading or handling techniques) and/or procedural factors (such as lens and inserter interaction) might have contributed to the event.
