It was reported that a patient line extension set leaked during peritoneal dialysis therapy.This event occurred during dwell five of seven while the patient was connected.The technical service representative (tsr) assisted the customer in ending therapy.In a follow-up call, the patient stated that the leak was coming from the tubing of the patient line extension, near the top of the tubing by the connector.The patient stated that there was a small hole or crack from which the leak was slowly flowing.The patient did not have any pets or animals that could have caused the damage.The patient did not use any sharp objects when opening the shipping box or the individual pouches.The patient stated they were sent to the hospital to get prophylactic antibiotics due to the potential contamination exposure.There was no patient injury or medical intervention associated with this event.No additional information is available.
