On (b)(6) 2016, the patient was implanted with a gore® acuseal vascular graft into the brachiocephalic artery to perform an arteriovenous access.On (b)(6) 2017, the patient presented with a recurrent thrombosis of the graft.It was reported to gore that during a closer inspection of the thrombosis via an ultrasound scan, that the inner layer of the graft was dissecting at the distal end of the graft towards the venous end, as if the silicon layer was coming away, causing outflow obstruction.A thrombectomy has been performed and an attempt at radioplasty.It has been decided to leave the graft in situ and implant another graft.No consequences were reported for the patient.
