It was reported that during the implant procedure, there was difficulty getting stable left ventricular (lv) lead thresholds.The lead was placed in a lateral sub vein of the coronary sinus and repositioned several times to attempt stable threshold values, however, the physician began to suspect the lv lead penetrated the vein wall at some point, which was confirmed by contrast injection.After contrast injection, the patient complained of pain in the throat and back and had a fall in blood pressure.A pericardial drain was performed and it was noted there was cardiac tamponade observed from the perforation, along with a pericardial effusion.The patient was taken to the (b)(6) lab for further follow-up and the lv lead placement was abandoned.The lv lead was removed.No further patient complications have been reported as a result of this event.
