(b)(4).Every time the customer tired to use the device they had to use tape to help it stick.They couldn't get them to stick so they took them off.They tried 13 level sensor pads before they found one that would stick.

Event Description

It was reported that during set-up of the device for a cardiopulmonary bypass (cpb) procedure, the customer can't get the pad stick to the reservoir.As a result, an alternate device was employed.The surgical procedure was completed successfully.There was no delay, no blood loss, nor adverse consequences to the patient.

Per follow-up with the user facility perfusionist, the sensor pads are mounted on an area of the reservoir that is free of labels or seams in the plastic.The flattest possible location is always used for application of the sensor pads as they understand it results in the least amount of strain on the pads.The pad was applied immediately.Several of their perfusionists have stated that they will warm the area where the pad will be applied by different means and that seems to help.

The reported complaint was confirmed.No product was returned to the manufacturer for evaluation.A full case replacement was sent to the customer.The manufacturer's territory manager instructed the users at the facility on how to put on the level sensor pad.They have had no issues since then.If additional information becomes available on this complaint that would alter the facts and/or conclusion, a supplemental report will be filed accordingly.
